My fire dept. is trying to create a suitable medical assist report. I surfed the web for sample copies but only came up with ones suitable for ems care givers. Our level of care isn't that advanced....only a few at the First Responder level. Before I go and modify what we have with what I've found I'd like to see if anyone else has a report they are willing to share.
M.A.R., pre hospital care report, first responder care report...whatever you call it I'll gladly accept samples.

I will post a copy of the BC first responder report for you later today. I gotta run on a call at the moment but it might be good for what you're looking for.

I don't have copies or anyhting like that to look at, but we have first responder forms, given to us by the government. Maybe you guys have something similar?

Unfortunately the BC first responder report I have as a pdf is too large to upload here. I will try and find a link I can post.

PFD023, I see you are in Ontario. You might what to consider a spot to record a DNR form number. The biggest thing for us is once the defibrillator gets turned on, you have crossed a line that requires dilligent documentation. PM me if you need anything futher.

I just came across this thread now. I'll see if I can get a copy of ours to review. Our Mar is provided by Base Hospital.

Ahhhh but his neighbour's eldest son poured acid on them.......you guys should have had a few calls about that by now.

Oh sure!! the guy asks a legit question about M.A.R.S and you take 2 years to get back with an answer, so then he HAS to switch depts and now you drag me into your tea party.:eek:

Pardon me if I am mistaken but the last time he was swinging from trees wasnt he off work for a while :)

PFD023, was this REEEEALLY taken care of, or do you just not care now that you are...like you said...not with that dept? Seems to me we are working on coming up with a simplified MAR to replace the old county forms...still...two years after your original post, haha. I believe this is a "Partners" project, but have no fear, we are close to a solution.

The Chief actually did take one of the forms I presented and implemented it....scouts honour!
Probably too many big words for some of our neighbouring friends to read.....hard to "clap it out"with gloves on......and it was in english.
