When you have certain safety issues that keep being avoided by management and city hall, what is the next step? Who do you talk to?

I presume you have tried going the chain of command route. Do you have a functioning Health & Safety Committee? If not have you tried approaching your local with your concerns? Your final course of action would be your provincial OH&S Department.

In ontario the next step would be to call the Ministry of Labour.

The concern must follow the Internal Responsibilty system, reporting up, if the issue still exists it would be the same as refusing to do unsafe work outside of the fire service, call your provincial OH&S office and they will send an OH&S Officer to investigate. Take notes, if he visits;l accompany him and take pictures. CYA. There is a Whistle Blowers rule in most jurisdictions to protect you from reprecussions from management, city hall etc.
The best solution would be to fix yourself if possible, Due Diligence.

After exhausting the chain of command and your H&S commitee if you have one thre simple letters.......MOL.
BTW all you guys and gals in Ontario i am hearing the MOL is now not just for fire scenes, they are actually dropping into halls unannounced.

Yup, the past two years they hired 250 new MOL inspectors and with construction dying down in Toronto atleast alot fo them have more free time on their hands. They have already visited multiple sites of mine. Alot of instant 250-500 dollar fines coming out. They need to recoup the costs of all those inspectors :)
